ENROLLMENT
Steps:
- Secure enrollment form from the respective departments.
- Fill out the enrollment form legibly before securing approval from the principal/dean/program coordinator.
- Submit the enrollment form and the required documents to the Registrar’s Office for validation (Window 1 for High School, Windows 2,3,4,5 for College).
- Proceed to the Finance Office for the assessment and payment of fees (Window 11 and 12 for ISAP /Window 13,14,15 for MCNP).
Requirements:
Freshmen
- Form 138 / Grade 12 report card
- Form 137-A / SF-10
- Certification of Good Moral Character
- PSA Birth Certificate (clear photocopy), Marriage Certificate if married (for females)
- 2 copies 2×2 ID picture (with white background and name tag)
Transferee
- Honorable Dismissal (Transfer Credential)
- Official Transcript or Records
- True Copy of Grades
- Certification of Good Moral Character
- PSA Birth Certificate (clear photocopy), Marriage Certificate if married (for females)

- Police Clearance
CROSS ENROLLMENT
Steps:
- Secure cross-enrollment form from the Registrar’s Office (Window 1 for High School, Windows 2,3,4,5 for College).
- Fill out the enrollment form legibly before securing clearance from dean/program coordinator.
- Submit the accomplished cross-enrollment form to the Registrar’s Office for the issuance of the permit to cross-enroll.
- Submit the original copy of the assessment form from the school where the subject is to be enrolled.
Requirements:
For cross-enrollment to other schools:
- Provide the class schedule of the subject to be cross-enrolled to other school and ensure that it is not conflict with the current class schedule.
- Submit a copy of grades of the cross-enrolled subject/s to the Registrar’s Office after completion of the subject.
For cross-enrollment from other schools:
- Provide a permit to cross-enroll duly signed by the school registrar.
APPLICATION FOR GRADUATION
Steps:
- Secure application form for graduation from the Registrar’s Office (Windows 2,3,4,5 for College).
- Fill out the application form legibly before securing clearance from respective dean/program coordinator.
- Return the application form to the Registrar’s Office for validation.
- Proceed to the Finance Office for the payment of graduation fee (Windows 11,12 for ISAP / Windows 13,14,15 for MCNP).
- Submit the accomplished form to the Registrar’s Office.
Requirements:
- Attach a 2×2 ID picture in uniform attire with white background and name tag to the application form.
- Ensure the completeness of grades and required admission documents.
COMPLETION OF GRADES
Important Notice: Students must be on active enrollment and subject/s to comply is/are within the validity period.
Steps:
- Secure completion form from the Registrar’s Office (Window 1 for High School, Windows 2,3,4,5 for College).
- Fill out the completion form legibly before securing clearance from respective subject teacher and principal/dean/program coordinator.
- Return the completion form to the Registrar’s Office for validation.
- Proceed to the Finance Office for the payment of completion fee (Windows 11,12 for ISAP / Windows 13,14,15 for MCNP).
- Submit the accomplished completion form to the Registrar’s Office.
Requirements:
- Attach the requirement / exam paper to the completion form.
REQUEST FOR ACADEMIC CREDENTIALS
Steps:
- Secure request form from the Registrar’s Office (Window 1 for High School, Windows 2,3,4,5 for College).
- Fill out the request form legibly before securing clearance from respective department and library.
- Proceed to the Finance Office for the payment of request (Windows 11,12 for ISAP / Windows 13,14,15 for MCNP).
- Submit the accomplished completion form to the Registrar’s Office.
Requirements
- Certification of Enrollment / Grades (processing time: 3 working days)
- Ensure enrollment status is active and grades are complete via student account.
- If student account is unavailable, proceed directly to Registrar’s Office.
- Graduates’ Credentials / Other Certifications (processing time: 7 working days)
- For 1st copy requests: Surrender school ID to the Registrar’s Office
- For school requests: Provide an official school request letter indicating the required documents.
- Note: Have a graduation or recent formal picture be made available when requested.
- Academic Verification (processing time: 7 working days)
- Provide a clear printed academic verification in a short bond paper.
- Ensure all required fields to be filled out by the graduate/student are accomplished.
- Transfer Credentials (processing time: 7 working days)
- Ensure grades are complete via student account.
- If student account is unavailable, proceed directly to the Registrar’s Office.
- Surrender school ID to the Registrar’s Office.
- Certification, Authentication and Verification (CAV) (processing time: 20-30 minutes)
Provide the following documents upon request:- For Qatar: 1 set original credentials and 1 set of photocopied credentials in a long bond paper and reduced by 85% (Official Transcript of Records, Diploma, RLE for Nursing, RCE for Midwifery, Certification of Total Units Earned for undergraduates).
- For other countries / PNP application: 2 sets of photocopied in a long bond paper and reduced by 85% (Official Transcript of Records, Diploma, RLE for Nursing, RCE for Midwifery, Certification of Total Units Earned for undergraduates).
